The Annunciation Cathedral is pleased to assist parishioners and families with official copies of sacramental records, including Baptism, Chrismation, Marriage, and Funeral documentation.

To ensure timely and accurate processing, please complete the request form below with as much detail as possible.

Before You Begin

To help us locate your record efficiently, you will be asked to provide:

Full name(s) associated with the sacrament
Date of birth (if applicable)
Type of sacrament requested
Approximate date of the sacrament
Names of parents, sponsors, or spouse (depending on the sacrament)
Any additional details that may assist our records team

You will also have the option to upload supporting documentation if available.

Request Process

All requests are reviewed by our Cathedral office. Once submitted:

Requests are typically processed within 5–10 business days
You will be contacted if additional information is needed

Stewardship & Fees

As part of our ministry:

Documents are provided to Stewards of Annunciation Cathedral at no cost
Non-stewards may be asked to submit an administrative processing fee, to become a Steward visit www.annunciation.org/stewardship

Important Notes

Only records on file at Annunciation Cathedral can be fulfilled
Identification or verification may be required for certain records
